1. Registration and Academic Forms

  • The seminary provides standardized forms for course registration, transcript requests, and course evaluations.


  • Students must complete registration forms accurately and submit them within the established deadlines to ensure proper enrollment.


  • Transcript requests require completion of the official form and may include a processing fee.


  • Course evaluation forms are to be completed at the conclusion of each course to provide feedback on instruction, content, and overall learning experience.

 


2. Ministry and Internship Evaluation Templates

  • Students participating in practicums, ministry placements, or internships will use seminary-provided evaluation templates.


  • These templates are designed to guide supervisors in providing structured feedback on performance, skill development, and ministry competency.


  • Completed evaluations must be submitted to the seminary by the supervising mentor or ministry leader and will be incorporated into the student’s academic record.

 


3. Academic Appeal and Grievance Forms

  • Students may initiate appeals regarding grades, academic decisions, or disciplinary matters using official appeal forms.


  • Grievance procedures ensure fair review of complaints concerning faculty, staff, or fellow students.


  • Completed forms must include detailed documentation and supporting evidence and submitted within the timelines specified in the academic policies.


  • The seminary will review all submissions promptly, maintaining confidentiality and impartiality throughout the process.
